Student Transportation Form for Athletics & After School Programs

If your student will be participating in our after-school programs, please complete the form below. Please note, your student will not be dropped off at their home, you must select one of the following drop off locations with route color and number and the persons allowed to receive your student. If your student may be left at the stop to wait for you unattended, the attached Latchkey Form will be required. Please ensure that all phone numbers are current and that you notify Transportation if they change, so that we can reach you if needed.

The following is a list of locations and times for our after-school programs for the 2023-2024 school year. The buses leave at 5:45 P.M. Please arrive 10 minutes prior to the scheduled time, to pick up your student at the listed times. Due to traffic and tardy parents, the bus may be late getting to your stop. We can try to alleviate this by parental cooperation in being prompt in picking students up. The bus may also be early due to fluctuations in the number of students taking part in the after-school programs. Please try to be understanding.
